How to prepare

Small details, cleaner data

Follow any instructions given by your clinician or the drawing lab — they take precedence over general guidance.

Fasting

  • An 8 to 12 hour fast can improve comparability for lipids, ApoB, fasting insulin and glucose — and sometimes CMP.
  • Water is fine.
  • Avoid heavy exercise and alcohol the day before.

Supplements & Medications

  • Biotin can interfere with some immunoassays — ask your clinician whether it should be paused.
  • Note thyroid medications and their timing relative to your draw.
  • Note hormone medications, including testosterone or estrogen therapy.
  • Note statins and other lipid-lowering therapy.
  • Note GLP-1 therapy.
  • Note steroids and high-dose vitamins.

Do not stop or change any prescribed medication without direction from your clinician.

Time of Day / Cycle Phase

  • Morning draws are generally preferred.
  • Testosterone is typically drawn in the morning.
  • Cortisol, if ordered, is timing-sensitive.
  • Fasting measurements are easiest first thing in the morning.
  • Women's hormone testing may need cycle-specific timing when recommended by a clinician.

Context, not a diagnosis

Cardiometabolic Risk

These markers are read together as a pattern rather than individually, and always alongside your history and clinical picture.

  • ApoB reflects atherogenic particle count and can add context beyond LDL-C alone.
  • Lp(a) is largely genetic and usually informs long-term risk context rather than short-term change.
  • hs-CRP gives a signal of low-grade inflammation, sensitive to recent illness or intense training.
  • HbA1c reflects average glucose exposure over roughly three months.
  • Fasting insulin, read with glucose, gives context around insulin sensitivity.
  • The TG/HDL-C ratio is one of several simple context measures clinicians may reference.

Thyroid Balance

Thyroid results are interpreted as a set — a single value rarely tells the whole story.

  • TSH is the usual first-line signal of thyroid balance.
  • Free T4 reflects the available pool of the primary thyroid hormone.
  • Free T3 reflects the active hormone and is read with TSH and Free T4.
  • Antibodies, when ordered separately, can add autoimmune context.

Hormone Context — Men

Male hormone results depend heavily on draw timing and binding proteins.

  • Total testosterone is influenced by SHBG, so free testosterone adds context.
  • Morning draws are generally preferred for comparability.
  • DHEA-S provides adrenal androgen background.
  • Results are considered alongside symptoms, not in isolation.

Hormone Context — Women

Female hormone results are read against cycle phase or menopausal status.

  • Estradiol varies substantially across the cycle.
  • Progesterone, where applicable, is timing-dependent.
  • Perimenopause and menopause shift expected patterns considerably.
  • Your clinician may recommend a specific cycle day for repeat testing.
